Actually, I agree with pretty much everything Bros Tigerprawn and Ronee have said. So, to be clear. Orgasm starts when, with the buildup of pleasurable sensations, the woman loses control and climaxes. I think Bros Tigerprawn and Ronsee will pretty much agree with this definition.

It is easier for a woman to be brought to orgasm with genital stimulation, especially of the clitoris, and more difficult without. How much more? Depends on the girl, the guy and other factors.

That said, genital stimulation is not absolutely necessary. Without genital stimulation, the sensations during foreplay can also build to the point when your darling "buay tahan", at which point, she loses control and experiences all the sensations of orgasm, like genital contraction, pulsations and so forth.
